There is one generic form to be completed by applicants. Please ensure that documents sent are certified and all documentation required is submitted. Incomplete forms will not be considered and may considerably delay the process.

2013 Renewal of Licence
All practitioners need to apply for 2013 recertifcation to obtain the Annual Practice Licence. All applications with the prescribed fees must reach the Secretariat by 31 January 2013.
